Appointments

• If you arrive 15 minutes or later for a scheduled appointment, the service provider has the right to refuse service or reduce the length of the service to ensure that it does not interfere with the next scheduled appointment. This rule applies to all of our services and clients.

• If you cannot make your appointment, please call to reschedule or cancel at least 24 hours before your appointment start time. If you do not provide sufficient notice, you will be charged up to the full amount of your scheduled service.

• All children under the age of 18 are classified as minors and must have a parent/guardian signature on their intake form. All children under the age of 12 must have a parent present during their session. Rejuvenate Body Spa cannot be responsible for minors while left unattended. Some state regulations require a parent or guardian to be physically present in the room during the treatment of any age minor

Gift Certificates and Gift Cards

• Gift certificates and gift cards are not redeemable for cash.

• We cannot accept or verify lost gift certificates and gift cards.

• All gift certificates and gift cards must be presented at the time of service or you must pay by other means.

• All gift certificates and gift cards are non-refundable.

• Rejuvenate Body Spa cannot be held responsible for lost or stolen items.

It is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success or effectiveness of individual techniques or series of appointments.

I understand that massage therapy is not a substitute for a medical examination, diagnosis, and/or treatment.

I understand that the treatment I receive for the purpose of stress reduction and relief of muscular tension, spasm, or pain, and to increase circulation.

If I experience pain or discomfort at any point during the massage, I will immediately inform the massage therapist so that the pressure or methods used can be adjusted to my comfort level.

1. Prior to a massage, remove all jewelry and pull long hair back.

2. Please provide feedback as to pressure (soft, medium, strong) and discuss painful or ticklish areas.

3. Feel free to ask questions about the procedures. The massage provider is well trained, ethical, and professional, and will be happy to make you feel well informed and comfortable.

4. Any illicit or sexually suggestive remarks or advances will result in immediate termination of the treatment.
